Vanshika Ajay Shah.

Abstract
Adverse drug reactions are very commonly occurring events in the field of medicine causing a great degree of discomfort to patients and their families likewise. They are essentially an unintended harmful effect of a standard dose of medicine which is unprecedented. They may be occuring due to drug drug interaction, Drug food interaction, comorbidities etc.
A major drawback faced is that these adverse reactions are not reported adequately. As they are unpredictable their reporting assumes greater importance to create a database of such adversities and a general sense of awareness which can help control them. This place highlights the common limitations due to which areas are underreported. Some of them being - legal repercussions, losing reputation, poly pharmacy, complex reporting procedures, time constraints, lack of awareness, insufficient training, unaware patients etc. However all of these challenges can be overcome with proper management, training and a general culture of reporting.

Key words: Adverse drug reactions, pharmacovigillance, underreporting, challenges and roadblocks in reporting, drug surveillance, patient safety
